[RUME] Survey instrument for writing in math

Can anyone direct me to resources (instrument or related literature) for
surveying a mathematics department (faculty and/or students) about the
mathematical writing students do, regarding the
1. type (e.g., writing solutions to non-proof problems (i.e. Calculus
textbook exercises), solving problems on exams, doing projects with a
written component, writing proofs, writing papers, posters),
2. amount and frequency
3. quality. 

We would also like to query how each of these forms of writing is evaluated:
Graded for accuracy? Completeness only?  Not collected? Graded against
mathematical writing standards (such as those in Price, 1989)?

Thanks for any suggestions or insights on this task we would like to
undertake.
